STEP 1-  EVALUATION OF DOCUMENTS

• Completely filled-out Senior High School Application Form

• Photocopy of First Quarter Grades

• Recommendation from the previous school

• Good moral certification

• Baptismal Certificate

• N.S.O. Birth Certificate

• Medical Certificate

• 4pcs. 2×2 pictures

N.B. Said documents must be placed in a long brown envelop. All these documents shall become the property of the Admissions Office.

STEP 2- WRITTEN EXAMINATION

• The applicant pays at the Finance Office the entrance fee of Php 100 One Hundred only. Receipt of which must be presented to the Admissions Officer.

• The applicant takes the entrance exam.

STEP 3 – PERSONAL INTERVIEWS

• Guidance Counselor

• Associate Principal

• Principal

STEP 4- DECISION

• Qualified applicants will be given an Enrollment Acknowledgment Slip/ Cumulative Records Folder

SENIOR HIGH SCHOOL

ENROLLMENT PROCEDURE

STEP 1-   Present the Enrollment Acknowledgment Slip to the Office of the Registrar.

STEP 2-   Fill – out the Enrolment Form and submit to the Office of the Principal for approval/signature

STEP 3- Present duly signed enrolment form to the Finance Office for assessment and payment of fees. (Books and uniforms may also be purchased at the Finance Office).

STEP 4-   Submit enrolment forms at the Office of the Registrar.The Registrar  will take charge of the distribution of the forms.  (Student, Office of   the Principal, Registrar and Finance).
